How this website
handles your information.

Who we are

This website, sytekortho.com, is operated by Sytek Ortho, PLLC, 1132 W. Chisholm St., Suite A, Alpena, MI 49707. You can reach us at 989.340.0671 or info@sytekortho.com.

This policy explains what the website collects and what we do with it. It is not the same thing as your health record. If you are a patient, the information we hold about your care is covered by our Notice of Privacy Practices, which is the document that sets out your rights under HIPAA.

Information you give us

We only collect what you type into one of our forms.

Contact form. Your first and last name, email address, phone number, what your question is about, and your message.

Consultation request. Your name, phone number, email address, whether the appointment is for you or your child, the patient’s age, how you prefer to be contacted, and anything you choose to tell us.

Referring dentist form. Intended for dental professionals referring a patient to us. It asks for the referring dentist’s name and office email address, the patient’s name, phone number and email address, and optionally a parent or guardian’s name where the patient is a minor. We deliberately do not accept x-rays, chart notes, dates of birth or any other clinical detail through this website. If we need records, we contact the referring practice directly.

You do not have to use any of these forms. You are always welcome to call the office instead.

Information collected automatically

Our website does not set advertising or tracking cookies, and it does not build a profile of you.

Two things happen automatically that are worth naming:

Our web host keeps standard server logs, which record things like your IP address, browser type and the pages requested. These are ordinary technical records used to keep the site running and secure.

Our forms record how you arrived at the site, for example which page you came from or which search brought you. We use this only to understand which parts of the website are useful.

Google Fonts and Google Maps. Our pages load typefaces from Google, and the map on our contact page is embedded from Google Maps. Loading them tells Google your IP address, in the same way visiting any website tells that site your IP address. Their handling of it is governed by Google’s own privacy policy.

Website analytics. We use Google Analytics to understand how many people visit the site and which pages they read. It tells us about visits, not about you personally: we do not use it to identify individual patients, and we do not combine it with your health record. Google processes that data under its own terms.

How we use your information

We use what you send us to reply to you, to book and manage appointments, to answer a referral from your dentist, and to keep records of the enquiries we receive.

We do not sell your information. We do not share it with advertisers. We do not use it to target you with advertising anywhere else.

Who else may see it

We use a small number of outside services to run the practice, and they see only what they need in order to do their job for us:

Our website host, which stores the site and the enquiries submitted through it.

Our email provider, which delivers form submissions to the office.

OrthoFi, which handles new patient paperwork, financial agreements and payment plans. If you create an OrthoFi account, their own privacy policy covers what happens there.

Ortho2, which provides our online appointment booking. Choosing a time online takes you to their system.

We may also disclose information where the law requires it.

How long we keep it

Enquiries sent through the website are kept on the website server for 30 days and cleared monthly. Anything we need beyond that is entered into our practice management software, which holds it under stronger encryption and access controls, and the website copy is deleted. Patient treatment records are kept separately, for the period required by Michigan law and our professional obligations.

Children’s privacy

We treat a great many children, so parents and guardians use this site on their child’s behalf. The website is intended for adults. We ask that a parent or guardian, rather than a child, completes our forms.

How we protect your information

The website is served over an encrypted connection, and access to the enquiries it receives is limited to the people in our office who need it.

Please do not send detailed medical information, images or anything else sensitive through the website’s general contact form. Email is not a secure way to send health information. If you need to send us something confidential, call the office and we will give you a secure way to do it.

Your choices

You can ask us what information we hold about you, ask us to correct it, or ask us to delete it, by calling 989.340.0671 or emailing info@sytekortho.com. Requests about your health record are handled under our Notice of Privacy Practices instead, which gives you additional rights.

Changes to this policy

If we change how the website handles your information, we will update this page and change the date below.
